Talk to Adam in his house at the North end of Wendlewick about Hearts of Sanguine. He will ask what visions you saw. Accept the quest and you will relay what you remember. He will confirm that since these visions came from Inanna they should be taken seriously. He'll ask you to investigate and will suggest you start by speaking to Raz and Bartender Gefen.

Bartender Gefen

Head South of Adam's house to the pub by the river and talk to Bartender Gefen inside behind the bar. Choose to discuss the quest and you'll ask her if she's heard about anything strange going on recently. She will tell you that there's a newcomer to Wendlewick who mostly keeps to herself. She has been seen coverting with some of Gefen's regulars: Jacob, Matthew, and Miriam. Before you continue, trade with her to buy a Wendlewick ale.

Jacob: Talk to Jacob at the table directly outside the pub about the quest. From him you'll learn the woman has pointed ears and isn't human--he assumed she's an elf.

Matthew: Talk to Matthew at the table just inside the door of the pub about the quest. He's definitely drunk, but says he'll tell you what he knows if you get him a Wendlewick ale from Bartender Gefen first. Give him the ale and you'll learn that something about the woman's behavior feels like a predator restraining itself.

Miriam: Talk to Miriam by the counter outside the pub about the quest. You'll learn the woman has delicate, pointed canines.

Jacob: Talk to Jacob at the table directly outside the pub about the quest. From him you'll learn the woman has pointed ears and isn't human--he assumed she's an elf.
- Talk to Bartender Gefen again to relay what you learned. She will suspect you are dealing with a vampyre. Complete the dialog to finish this part of the quest.
Raz

Travel North West from Adam's house for a few minutes until you reach Raz's home by the lake. Talk to Raz about the quest. You will ask her if anything strange has been going on in the forest and she'll tell you she's found some peculiar animal carcasses on her daily stroll.

Loop East around the ridge South of Raz's home and travel North toward the Southern log balance over the river. Just before you reach the log you should encounter the body of a dead bear by the edge of the river. Inspect it to learn that it was killed recently, but its body is bone dry, and there are 2 deep puncture marks on the neck. Continue the dialog to begin a cutscene in which it will become apparent you are being watched.

Return to Raz in her home and talk to her about the quest. She will suspect you are dealing with a vampyre. Complete the dialog to finish this part of the quest.

Once you've finished up with both Bartender Gefen and Raz, return to Adam's house and talk to him about the quest to fill him in on what you've discovered. He'll say he needs to prepare Zeke and the rangers accordingly, but in the meantime Rachel's sheep have been going missing under odds circumstances that could be related. He'll ask you to look into it.

Head South East, cross the river, and make your way to Rachel's farm. It's the only farm with sheep, and can be found North East of the only fairy ring in Wendlewick. Talk to Farmer Rachel in the house about the quest. She will mention a voice that says it's too late. You will promise to investigate.

Exit the house and go North to the pen that's been broken open. Follow the blood trail North East to a cave and enter it to begin a cutscene. The mysterious woman who was watching you inspect the bear will appear and have you step back from the beating heart in the center of the room. She will introduce herself as Anya. She will also claim that the blight in this cave not only isn't her doing, but that combating it is her life's work. Explore the dialog as much as you like, but choose the final option to continue the quest. She'll tell you this phenomenon is the Sanguine, which lures creatures and consumes their blood to grow stronger until none can resist its call. A potion will be needed to resist it, so you will mention Esther. Anya will ask you to bring her to meet Esther.

While you can walk back to Wendlewick, it's fastest to just teleport to the town lodestone; Anya will still follow you. Run South to the lighthouse, climb the stairs on the South side to the middle floor, and speak to Esther about the quest. Anya will give Esther a mourning lily, which she says is poisonous to her kind but also resists the Sanguine. Esther will take it and come up with a recipe for a potion to resist the power of the Sanguine. To make the potion you will need the following:
- Hedgehog spines
- Poppy
- Wolf's tongue mushroom

You can gather the ingredients in any order, but we will get them alphabetically.

Hedgehog spines: Make your way South West of the shrine to Inanna and you will find several hedgehogs. They don't appear on the map, but simply kill one and it will drop the spines you need.

Poppy: Head towards the only bank marker on the South side of the island. Just North of it, climb the large ridge to the West. Near the top you will find a standing stone with poppies at the base that you can pick.

Wolf's tongue mushroom: Travel to the Wendlewick fairy ring (code BKS) on the Southern island. Pick a Wolf's tongue from any of the spots nearby.

Hedgehog spines: Make your way South West of the shrine to Inanna and you will find several hedgehogs. They don't appear on the map, but simply kill one and it will drop the spines you need.

Return to Esther on the middle floor of the lighthouse and talk to her about the quest again. She will take the ingredients and give you an Antisanguine recipe. You're going to make the potion now. The room is circular, so we'll be describing the location of objects using the clock model: North is 12:00, East is 3:00, etc.

Take a Vial from the vial cabinet (12:30)

Fill the Vial from the Vat of oil (6:30) to obtain a Vial of oil.

Sterilise the Vial of oil at the fireplace (2:00) to obtain a Vial of oil (s).
-
Take from the bucket of hedgehog spines (4:00), sack of flowers (5:00), and sack of mushrooms (5:30) to reobtain the 3 ingredients you brought to Esther.

Grind the Hedgehog spines into Ground hedgehog spines, then use them on the Vial of oil (s) to create Antisanguine? in the interface. In your inventory it will become a Vial of oil (crushed hedgehog spine).

In the center of the room, press the Wendlewick poppy on the table to obtain a Pressed Wendlewick poppy.

Use the Pressed Wendlewick poppy on the Vial of oil (crushed hedgehog spine) to create Antisanguine? in the interface. In your inventory it will become a Vial of oil (crushed hedgehog spine, pressed Wendlewick poppy).

Go to the desk (10:00) and weigh out 30g of wolf's tongue to obtain Wolf's tongue (30g).

Use the Wolf's tongue (30g) on the Vial to create Antisanguine? in the interface. It will become Antisanguine in your inventory.

Take a Vial from the vial cabinet (12:30)

Talk to Esther about the quest again. Anya will suggest testing the Antisanguine on some of the woodland animals to figure out the proper dosage for you to take.

Descend to ground level and make your way toward the fairy ring, then travel South until you reach the lake. Upon getting close to the hedgehog by the North West shore, Anya will point it out. Use the "test antisanguine" option and it will attack you. Kill it an complete the dialog.

On the South Eastern shore of the lake will be a Duck with the same option. It will grow larger until it explodes. Gross.

Go a short distance North towards the campsite (Anya says its hers) and you'll find a Badger with the same option. Test it and Anya will say the Badger successfully retained some resistance to the Sanguine. You will ask her next about obtaining a weapon that could harm the Sanguine heart. She'll tell you to obtain the purest silver you can to make a sword that could do the job, then meet her back at the cave.

Make your way to the Wendlewick smithy and talk to Liat. She'll say that there's a unique kind of silver in Havenhythe called Havensilver. If you can help her with her ore shortage, she'd be willing to part with what smaller pieces of Havensilver she has to melt down and make a weapon.

Follow the path North of town until you reach a fork in the road next to an abandoned cart. Take the Eastern path and follow it to the mine, where you'll find Gidon. Talk to him about the quest and he'll request that you get the next ore shipment done for him: 10 Copper and 10 Tin ore. With the time that saves him, he might be able to get into the buried tunnel he's discovered. If you didn't bring the ore yourself, you will need to mine it before you continue.

Return to Liat in the smithy and talk to her to give her the ore she requested. She'll say she has some orders to complete before she can help you with the Havensilver, so in the meantime you offer to check on Gidon's progress breaking into the tunnel.
Note: She will not accept bank notes, the ores must be unnoted.

Return to the Northern mine and you'll find the entrance to the tunnel is open. Go through it and talk to him. He'll say there's several untapped veins of Havensilver by the stairs. Mine 4 Havensilver ores, then teleport back to Wendlewick.

Return to the smithy and talk to Liat. She will teach you how to smith with Havensilver. Use the furnace to smelt the ore into 4 Havensilver bars, then use the forge and anvil to smith a Havensilver greatsword.

This is your last chance to prepare for the fight. Melee armor is recommended and players with less than 20 combat should consider bringing food. When you're ready, meet Anya outside the cave North of Farmer Rachel's sheep pens. The three leaders (Adam, Esther, and Zeke) will be there as well. Talk to any of them.

Lower level players should read this entire step before continuing. When you're ready for the fight, enter the cave.
As soon as you enter the cave, drink your Antisanguine. It will protect you from Sanguine attacks for 5 minutes. Attack the Sanguine heart. Several times throughout the fight it will spawn Sanguine tumours. While any of them are present, it is immune to damage. If your Antisanguine isn't active, any damage you would deal to it is instead reflected back at you. Each time the tumours appear, stop attacking the heart and kill them. Occasionally a pustule on the heart will explode, splashing on the ground where you are standing. If you don't move away quickly, it can immediately remove the effects of your Antisanguine potion. Each time this happens, just drink more: the potion doesn't run out of doses. When its hitpoints reach 1, it will try to invade your mind and Anya will push you to deal the final point of damage.

Exit the cave and talk to Zeke or Anya. Pick any of the chat options you like, then complete the dialog.

Return to Adam's house and talk to him about the quest to finish up.
